Patient List Report

The Patient List Report will provide you with a variety of patient information depending on the way you filter the report and the information you have in your system. Depending on the report, you can pull all kinds of patient demographics. See below for what is included in each report.

How to run the Patient List Report

1. From the VitaLogics Main Menu Click on Reports

2. Select Patient List

3.  Here you will have several filters to choose from:

  • Patient Status type: You can select Active, Inactive or Terminated. You can also select more than one of these options by holding down your CTRL button on your computer and select each status until what you want is highlighted
  • Providers: If you want to select ALL providers - you can click on "Check for all providers" OR you leave that box Unchecked and you can select specific providers. Holding down the CNTRL button and select providers.
  • You have a choice of report type: (More detail for each report type will follow below)

 

PROFILE REPORT 

(sample below)

  1. Select all your filters
  2. Select PROFILE REPORT
  3. Click on NEXT
    • Profile Report - will include Prospect, Lead and Patient info and has the most patient demographics: name, nickname, address, work, home & cell phones, email, birthdate, sex, marital status, new patinet date, SSN, spouse, employment status, employer, occupation, status, account#.
    • This report can be downloaded to Excel, PDF or Word 
    • You can also create mailing labels if needed from this screen - just click on MAILING LABELS on bottom right corner BEFORE you finish

LIST REPORT 

(sample below)

  1. Select all your filters
  2. Select LIST REPORT
  3. Click on NEXT
    • List Report - will include Prospect, Lead and Patient info as well as Fist name, last name, middle initial, address, home & work phone, email, birthdate and status
    • This report can be downloaded to Excel, PDF or Word
    • You can also create mailing labels if needed from this screen - just click on MAILING LABELS on bottom right corner BEFORE you finish

EMAIL REPORT 

(sample below)

  1. Select All your filters
  2. Select EMAIL REPORT
  3. Click on NEXT
  • Email Report - will include Prospect, Lead and Patient info as well as First Name, Last Name and Email address
  • This report can be downloaded to Excel, PDF or Word
  • You can also create mailing labels if needed from this screen - just click on MAILING LABELS on bottom right corner BEFORE you finish

CSV 

(sample below)

  1. Select All your filters
  2. Select CSV
  3. Here you can select any or all of the following: PROSPECT, LEAD, PATIENT
  4. Click on NEXT
  5. This will take you to a SAVE AS screen so you can select where you would like this CSV file to be saved
  6. Click on SAVE
  7. You can find your CSV file where you saved it
    • CSV file - will include First Name, Last Name, Address, Home Phone and Work Phone, Email and Status
