Patient List Report

 

The Patient List section houses 4 different reports. The Profile Report, List Report, Email Report and the ability to create a CSV file.

Each report will provide you with a variety of patient information depending on the way you filter the report and the information you have in your system.

See below for what is included in each report.

 

Under Reports

Patient List

 

Here you will have a couple filters to choose:

 

  • Patient Status type: You can select Active, Inactive or Terminated. You can also select more than one of these options by holding down your CTRL button on your computer and select each status until what you want is highlighted
  • Providers: If you want to select ALL providers - you can click on "Check for all providers" OR you leave that box Unchecked and you can select specific providers. Holding down the CTRL button and select providers.  

 

 

You will need to make sure the proper filters are selected before selecting Any of the report options

 

Then you select the specific report to run

 

PROFILE REPORT

  • Will include Prospect, Lead and New Patients (Based on Practice Diagnosis report patient flow)
  • Has the most patient demographics: Patient name, nickname, address, work, home & cell phones, email, birthdate, sex, marital status, new patient date, SSN, spouse, employment status, employer, occupation, status, account# and VitaLogics Internal ID provided by software.
  • This report can be downloaded to Excel, PDF or Word
  • You can also create mailing labels if needed from this screen - just click on MAILING LABELS on bottom right corner BEFORE you finish

 

 

LIST REPORT

  • Will include Prospect, Lead and New Patients (Based on Practice Diagnosis report patient flow)
  • The List Report will include the patients Last Name, First Name, Middle initial, address, home & work phone, email, birthdate and status
  • This report can be downloaded to Excel, PDF or Word
  • You can also create mailing labels if needed from this screen - just click on MAILING LABELS on bottom right corner BEFORE you finish

 

EMAIL REPORT

  • This report allows you to generate a List of ALL patients in VitaLogics and their email addresses. This allows you to export those email addresses, so you can upload to an outside emailing sources for bulk emailing etc.
  • Will include Prospect, Lead and New Patients (Based on Practice Diagnosis report patient flow)
  • Will display First Name, Last Name and Email address
  • This report can be downloaded to Excel, PDF or Word
  • You can also create mailing labels if needed from this screen - just click on MAILING LABELS on bottom right corner BEFORE you finish

 

 

To Print, Download or create Mailing labels on any of the above Reports

 

All reports will display a tool bar at the top of the page when generated. You will be able to Print and Export the document. You will see the Red Arrow is pointing to the Export options. X=Excel, A=Adobe and W=Word.

 

 

To create Mailing Labels, select the mailing label button at the bottom right of the screen.

 

 

 

CSV

This report is available, so a clinic can pull a report of their Patient Demographics. This is typically used if a client is moving to another software.

  1. Select All your filters
  2. Select CSV
  3. Here you can select any or all the following: PROSPECT, LEAD, PATIENT
  4. Click on NEXT
  5. This will take you to a SAVE AS screen, so you can select where you would like this CSV file to be saved
  6. Click on SAVE
  7. You can find your CSV file where you saved it 

The CSV file will include Demographics Only First Name, Last Name, Address, Home Phone and Work Phone, Email and Status
